It was shown to us in season 4 both a Dragon flying over Kings Landing and a wrecked throne room, most people thought it was something to do with the NK though as they thought it was snow covered in the room not ash and nothing to do with Dany.

the problem wasn't that she went mad, but that the build-up wasn't good, like it was obvious what they were doing with the story and the writing was kinda poor - there wasn't the forethought that the earlier seasons had (or appeared to have)

 

it also seemed very out of character for the civilians to be killed

Mad thing was HBO were willing to keep going but the showrunners wanted to end it where they did. So it wasn't even due to it getting too expensive for them.
